The Advanced Practice Provider (APP) will work closely with our physicians and clinical staff to provide comprehensive care to adult patients, with an emphasis on preventive medicine, chronic disease management, and cardiovascular health. The ideal candidate will have strong clinical assessment skills, experience managing acute and chronic medical conditions, and an interest in providing long-term, patient-centered care within a collaborative multispecialty practice. Key Responsibilities Conduct comprehensive patient assessments, including medical histories, physical examinations, and evaluation of acute and chronic medical conditions. Diagnose and manage common medical conditions, including hypertension, diabetes, hyperlipidemia, and cardiovascular disease. Develop and implement individualized treatment plans in collaboration with supervising or collaborating physicians, as applicable. Order, review, and interpret laboratory tests, EKGs, diagnostic imaging, and other appropriate studies within the provider's scope of practice. Assist with cardiac diagnostic evaluations and coordinate additional testing, including stress testing, when clinically indicated. Prescribe medications and recommend therapeutic interventions in accordance with applicable New York State regulations and clinical privileges. Provide routine follow-up care, monitor treatment effectiveness, and adjust care plans as appropriate. Educate patients and families regarding disease prevention, medication management, nutrition, exercise, and lifestyle modifications.
