Job description
The Center for Health Equity and Community Wellness (CHECW) aims to eliminate racial inequities resulting in premature mortality, with a focus on chronic disease, by addressing the social and environmental factors that impact health. CHECW works to increase placed-based investments in priority neighborhoods with community programming and services based on epidemiology; influence and leverage the health system to promote whole-person care; and intensify the agency's approach to tackle social determinants of health and root causes of health inequities. CHECW is comprised of the Bureau of Brooklyn Neighborhood Health, the Bureau of Bronx Neighborhood Health the Bureau of Harlem Neighborhood Health, the Bureau of Queens Neighborhood Health, the Bureau of Chronic Disease Prevention, the Bureau of Equitable Health Systems, and the Bureau of Finance, Administration and Services. The Bureau of Brooklyn Neighborhood Health has provided oversight and leadership in programming and planning for the Brownsville Neighborhood Health Action Center with satellite programming and planning also occurring at the Bedford and Bushwick District Health Offices. The bureau will support programming, planning and research opportunities to address racial inequities resulting in premature mortality in Central Brooklyn. The Bureau of Brooklyn Neighborhood Health seeks to hire a Monitoring & Evaluation Manager to oversee the development and implementation of monitoring and evaluation activities for the Family Wellness Suites (FWS). DUTIES WILL INCLUDE BUT NOT BE LIMITED TO: - Lead the development and implementation of monitoring and evaluation frameworks to assess the performance, reach, utilization, and outcomes of Family Wellness Suites programs and services. - Design and conduct program evaluations using quantitative and qualitative methods, including the development of logic models, data collection tools, performance indicators, and evaluation plans. - Oversee the collection, management, validation, and analysis of program data; establish data quality standards and ensure the accuracy, completeness, and integrity of program reporting systems. - Monitor key performance indicators and participant outcomes across Family Wellness Suite locations, identify trends and service gaps, and generate actionable findings to support program planning and decision-making. - Prepare reports, dashboards, presentations, and data visualizations that communicate program performance and evaluation findings to leadership, program staff, and external stakeholders. - Collaborate with FWS program leadership, staff, and community partners to identify evaluation priorities, strengthen data collection processes, and support the use of data for program management and strategic planning. - Participate in standing FWS program meetings to foster a strong understanding of the program interests and challenges. - Coordinate with the FWS Director to implement an on-site schedule that offers regular rotation and on-site support and program observations across the three suites. - Provide technical expertise on monitoring and evaluation methodologies, performance measurement, and data management practices to support program effectiveness and accountability across all FWS staff. - Collaborate with the FWS Director in developing the narrative and securing the appropriate data necessary for grant proposals that aim to support the sustainability of the FWS programming across the three suites. PREFERRED SKILLS: - Strong knowledge of monitoring and evaluation principles, performance measurement frameworks, and program evaluation methodologies. - Experience designing and implementing evaluations for public health, maternal and child health, healthcare, community-based, or social service programs. - Demonstrated ability to collect, manage, validate, analyze, and interpret quantitative and qualitative data from multiple sources. - Experience developing performance indicators, logic models, data collec
