Alignment Health is seeking a compassionate, motivated Bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members throughout Fresno, CA. In this highly field-based role, you'll build trusted relationships with members, caregivers, and families while helping individuals adopt healthier habits, access community resources, and improve their overall well-being. If you enjoy working independently, connecting with people face-to-face, and making a direct impact in your community, we'd love to meet you. *Applicants must be comfortable conducting daily in-home visits and traveling throughout Fresno, CA. What You'll Do Conduct approximately 6-8 in-home visits daily throughout your assigned service area. Provide wellness coaching and health education to support behavior change and healthier lifestyles. Partner with Advanced Practice Clinicians and Social Workers to develop member action plans. Connect members with community resources and social services. Support member engagement, retention, and satisfaction. Coordinate care with internal clinical teams and external partners. Deliver health education materials to members, caregivers, and families. What to Expect Primarily field-based role with daily local travel. Monday-Friday, 8:00 AM-5:00 PM PST. Visits are scheduled geographically to maximize efficiency. Mileage reimbursement provided from door-to-door travel. Minimum Requirements: Experience: Bilingual Spanish, must be able to write, read, speak Minimum 1+ year of experience as a wellness health coach or educator Current, valid, unrestricted California Driver's License. Reliable personal transportation and ability to travel throughout the assigned territory. Ability to maintain automobile insurance in accordance with company requirements. Ability to work independently and remotely. Strong communication, organizational, time management, and relationship-building skills. Proficiency with Microsoft Office (Excel, Word, and PowerPoint). Knowledge of health, wellness, nutrition, exercise, and behavior change principles. Ability to maintain HIPAA compliance and confidentiality. Education: Associate’s degree or two years additional experience in lieu of education. Preferred Requirements: Senior care experience Bachelor's degree Medical Assistant License Why Alignment Health?
